Your staffing rating is only as good as your PBJ file.
CMS builds the staffing star rating from Payroll-Based Journal submissions. If a quarter is late, has too many days without a registered nurse, or fails an audit, the rating drops to one star. Missing agency or contract hours make a well-staffed building look short-staffed.
